We have exciting opportunities for a motivated and enthusiastic, Band 5, Mental Health Nurse/Occupational Therapist/Social Worker to join the Home Treatment Team. The team is based in Woodlands on the Ipswich Hospital site.

The service operates 7 days a week, is age inclusive and provides daily support and care for individuals in a mental health crisis who can be supported with intensive care at home. The service provides community based psycho-social assessments, safe management plans and onward referral; working collaboratively with family and carers. They also provide the bed management  ad gate keeping role within the local area and support service user's on discharge from the ward. You will be an excellent communicator, self-motivating and committed to delivering high quality standards of care. You will be a confident and experienced practitioner with excellent assessment skills and the ability to formulate clinical decisions and management plans and act on them quickly with support from the wider MDT team.

We are looking for a flexible ‘can do’ attitude, with a demonstration of curiosity and creativity and a willingness and drive to build good working relationships with other teams, services and agencies. You will have a minimum of 1 year post qualification experience, including recent experience in acute mental health and be able to demonstrate a comprehensive understanding of crisis intervention.

You will be key in the efficient delivery of the team; in improving response and management of referrals; in helping the team meet their key performance indicators and be proactive in helping explore and develop partnerships and pathways to improve patient experience and outcome. As this service operates 7 days a week; you will be required to work shifts. We will provide a full induction and have a genuine commitment to training and development.
