As a Band 5 Physical Health Nurse you will be responsible for ensuring that all service users have the appropriate health checks, be involved in the developing client centered Physical Health care plans, health promotion and education of healthy life style choices, outreaching service users with local resources to continue making healthy choices in their discharge location in the community, working closely with the Doctors to advise about Physical Health Interventions and any recommendations, attending Clinical Reviews to discuss cases, educating staff in physical health interventions, taking a lead on smoking cessation guidance and additional duties as specified.

Suffolk Rehabilitation and Recovery Service (SRRS) support individuals with severe and enduring mental illness. We are currently the only mental health rehabilitation setting within Norfolk and Suffolk Foundation Trust.
We are committed to helping individuals on the recovery pathway - this could be assisting individuals following a long term battle with mental illness or helping a newly diagnosed individual regain the living skills and provide a foundation to develop preventing future admissions.
At SRRS we have a 10 bedded unit offering 24 hour support. our support includes continues assessment of individuals capabilities with regards to planned daily living activities and assessments based on the recovery model which endeavours to assist a move on to independent or further supported accommodation.
We will provide a full induction on recruitment, on-going training and regular peer support. we have a genuine commitment to training and development with an extensive teaching matrix and protected time for managerial and clinical supervision supported by psychology input. we are proud of our culture of honesty, team involvement/ ownership to both patient care and job satisfaction

This update service is an annual subscription at a cost of £13 to you. This will enable the Trust to have instant online access to your DBS record, with your consent, and so will remove the need for you to have to apply for this repeat check again.
