We are re-modeling our Older Peoples Services across all teams in Great Yarmouth & Waveney Locality. We are looking for motivated and forward thinking individuals who would like to join our team to deliver high quality care for older people. Our services cover both organic and functional mental illness in older people.
We have an inpatient ward for dementia care and assessment and support onward discharge to suitable placements in the community. Our new service encompasses in reach and outreach support working closely with the Dementia Intensive Support Team and Community Teams in both Great Yarmouth and Waveney. We are focusing our new pilot service around supporting Care and Residential homes based on the Newcastle Model of positive behaviour support and also supporting individuals in their own homes.
We have a vacancy for a full time Senior Practitioner (this could be a Mental Health or General Nurse, Occupational Therapist or Social Worker) and are looking for caring, enthusiastic and committed individuals who have experience of working with people with dementia to join our CMHT in Northgate hospital Great Yarmouth.
We work with individuals, families and carers in their own homes and other settings in order to promote independence, prevent hospital admissions, stabilise situations and improve the wellbeing of service users and their carers.
We welcome individuals with dementia experience but importantly those who want to support people to live well with dementia or functional mental health difficulties in older age. We have an experienced and supportive team who will make you feel welcome and help you to develop your leadership and clinical skills in a friendly and cohesive way.

This update service is an annual subscription at a cost of £13 to you. This will enable the Trust to have instant online access to your DBS record, with your consent, and so will remove the need for you to have to apply for this repeat check again.
You must have appropriate UK professional registration.
